If you request the Capability Statement, we collect your email address and your opt-in preference. We do not collect protected health information, and no form on this site should be used to transmit patient information.
How we use it
We use the information you submit to respond to your inquiry, to provide the Capability Statement when requested, and, only if you opt in, to send occasional company updates. We do not sell your information, and we do not share it with third parties except service providers acting on our behalf to deliver these communications. Form submissions on this site are transmitted and delivered to us through a third-party form processing service.
Analytics and cookies
This site uses privacy-respecting, cookieless analytics to understand aggregate traffic. It does not set advertising cookies and does not use third-party tracking pixels. The site honors Do Not Track signals.
Retention
We retain inquiry information for as long as needed to respond and to maintain a record of business communications, and update-list subscriptions until you unsubscribe.
